This is a full-time, permanent remote position.
About the role:
You’ll support patients through outbound contact, update medical records, provide education about care coordination, and help monitor and report on health outcomes. No prior experience needed – we provide full training.
What you’ll do:
- Connect with patients via phone to provide care coordination support
- Update and maintain patient medical information
- Educate patients about their care options
- Track and report on monthly health outcomes
- Complete assessments as assigned each day
Work schedule:
Monday to Friday, fully remote. Work from anywhere with a reliable internet connection.
Rate: Competitive entry-level pay, details shared upon application.
Contract length: Permanent, full-time.
Skills/background needed:
- No prior experience required – we train you from scratch
- Background in healthcare, caregiving, or health-related certification (CPR, CNA, or similar) is preferred but not required
- Comfortable communicating with patients over the phone
- Reliable internet connection for remote work
- Ability to follow detailed instructions and meet daily task targets
